During the first quarter of 2026, Total Rehab Moorestown was under the ownership of Moorestown Operator Holdco, LLC, according to Centers for Medicare and Medicaid Services (CMS). The Burlington County nursing home has operated with Moorestown Operator Holdco, LLC as its owner since Jan. 8, 2024.

Total Rehab Moorestown is classified as a for-profit facility.

In that timeframe, the facility earned a two-star overall rating from CMS, ranking below the New Jersey nursing home average of 3.4. CMS scoring reflects inspection results, staffing measures, and resident care quality.

Data indicates the nursing home provided care for an average of 123 residents per day in Q1 2026, while offering 124 licensed beds. The facility does not operate as a hospital-based provider.

CMS data also show that Total Rehab Moorestown reported no fines or penalties within the first quarter of 2026.

Burlington County had a total of 13 for-profit nursing home facilities at the time.

U.S. Census Bureau estimates highlight continued national growth of older adult populations even as youth numbers decline. From 2020 to 2024, the U.S. population for those 65 and older rose 13%, while the population under 18 showed a 1.7% decrease. The older adult demographic has expanded greatly since 1920, increasing from 4.9 million to over 61 million in 2024.

Research from 2021 estimates nearly half of those ages 65 and up—about 28 million people—may need long-term services and supports. The analysis also projects that more than one-third might ultimately require care in a nursing home setting.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
